Good Faith Estimate Notice
At Continuum Surgery Center of Naples, INC., we are committed to transparency in billing and to helping patients understand the potential costs of their care.
Patients who do not have insurance coverage, or who choose not to use their insurance, are entitled to receive a Good Faith Estimate. This estimate outlines expected costs for services and items related to your care, based on information available at the time it is prepared.
You may request a Good Faith Estimate by calling or visiting our facility.
Please note: your estimate may not include unforeseen or unexpected costs that arise during treatment. Actual charges may be higher if complications occur.
If billed charges exceed the estimate, you may:
• Request adjustment to match the estimate
• Ask about options for negotiating your bill
• Initiate a federal dispute resolution process
Disputes must be filed within 120 days of the original bill. A $25 fee applies.
