Advance Directives
All patients have the right to take part in decisions about their own healthcare. They may also create Advance Directives — a healthcare power of attorney — to make decisions on their behalf, in line with their expressed wishes, if they become unable to make or communicate those decisions. This surgery center fully respects and honors these rights. However, unlike an acute care hospital, our facility typically does not perform high-risk procedures. Most procedures here involve minimal risk—though, as with any surgery, some degree of risk always exists.
Your physician will review the details of your procedure with you, including:
• Potential risks
• Expected recovery
• Post-operative care
In the event of a medical emergency during your treatment at this facility, it is our policy — regardless of any existing Advance Directive or instructions from a healthcare agent — to initiate resuscitation or other life-saving measures. You will then be transferred to an acute care hospital for further evaluation and care.
At the hospital, any decisions to continue or withdraw treatment will be made in accordance with your Advance Directive or healthcare power of attorney.
Please note: agreeing to this policy does not cancel or invalidate any existing Advance Directive or healthcare power of attorney you may have.
If you would like to complete an Advance Directive, official state forms are available at our facility.
